Project ObjectiveSupport implementation of evidence-based best practices for disease management in medical practice related to diabetes.

Project DescriptionThe goal of this project is to ensure clinical practices in the community and ambulatory care setting use evidence based strategies to improve management of diabetes. Specifically, this includes improving practitioner population management, increasing patient self-efficacy and confidence in self-management, and implementing diabetes management evidence based guidelines.

Fall 2017 Update

As of DY2Q4, the Diabetes project has completed 71% of project milestones. 

           

As a means of reducing short-term complications, BLHC will pilot group visits and shared medical appointments. Research has found that these strategies have led to improved outcomes for patients with diabetes.

Project Update For February 2017

A core part of impacting Diabetes is ensuring that providers are updated on the most recent evidence-based care guidelines.  The 2017 American Diabetes Association (ADA) on-line training for providers is available through the portal HWApps.  A total of 73 providers in our network have completed to date on the 2016 Standards.  Please visit HWAppps through the Bronx Health Access portal to access this module.   

 

Recruitment is on-going for the patient Diabetes Self-Management  Program (DSMP).  This 6-week workshop is a peer-led program.   Classes are highly participative with the goal of mutual support and building participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.  If you would like to refer patients to a program or would like to offer a program onsite, please contact April Osorio at aosorio1@bronxleb.org

Project update for September 2016

The next Diabetes Self-Management Program (DSMP) leader training is scheduled to take place October 19, 20 and 27,28, 2016.. Space is limited and it is first come, first serve. To register visit www.surveymonkey.com/r/DSMPLeader3 .

There are currently 20 peer leaders registered to participate and build skills to become facilitators of the DSMP program. Registrants represent patients and clients from the community.  ADA Standards online training is currently available via HWapps for easier access to primary care providers and partners.
